When submitting an FM Equipment Request for one of your students, please ensure you have all three elements below before submitting to Auditory Outreach.

1. Completed and signed Auditory Outreach form for FM Request.
2. A signed Permission-to-Share Parent/Guardian Consent form.
3. Student's most recent audiology report.
